Coordinating Care for a Personalized Experience at Every Stage

What is Care Management?
The Care Management team at Arch Health Medical Group assists our patients in obtaining coordinated care. Working closely with a patient’s primary care physician, Care Management staff assists patients in a variety of settings by coordinating their care to ensure that their needs are met.
 
Inpatient Care Managers 
HMO inpatients are followed closely by our Inpatient Care Manager who meets with patients during their Pomerado Hospital stay and coordinates the discharge needs of patients with our physicians. Discharge needs may include coordination of home health care services, specialized medical equipment rental and assistance with extended care facilities for longer term recovery.
 
Additional Services
  • For patients requiring additional recovery time in a Skilled Nursing Facility, the Care Management team provides active monitoring, including follow-up phone calls to ensure that our patients understand their discharge instructions, that their condition is improving, and that follow-up appointments are scheduled with their doctors. If there are any problems, early identification and communication with the patient’s physician helps to get the patient back on the road to recovery.
  • For patients with chronic medical conditions such as diabetes, there are a variety of Care Management services available. Our goal is to ensure that patients have the skills they need to best manage their condition. This involves providing condition-specific education so that patients become empowered, active participants in their care. We have an excellent education series for those who prefer to learn in a group setting. Early identification of problems leads to early intervention by our physicians and better care for our patients.
