Pacific Accountable Care is a proud participant of the Federal Medicare Shared Saving Program.
Pacific Accountable Care (PAC) is a partnership of health care providers who work together to improve the quality, coordination and efficiency of patient care. Our goal is to provide high quality clinical outcomes at a lower cost by aligning health care providers, removing barriers to the coordination of patient care and transforming the delivery of health care with a patient-centered focus. Pacific Accountable Care, LLC (PAC) was approved by the Centers for Medicare & Medicaid Services to begin services on January 1, 2017.
